Terran Through Milestones (“TTM,” “we,” “us”) is a home-health pediatric physical therapy practice serving families across San Diego County. This policy explains what information we collect through our website and our text message program, how we use it, and the choices you have.
Information We Collect
- Information you give us. When you contact us through the website, request an appointment, or sign up for updates, we collect what you provide, typically a name, email address, phone number, and whatever you choose to write in a message.
- Information from intake forms. When your child becomes a patient, we collect the information on our intake paperwork, including caregiver contact details and, if you consent, a mobile number for appointment reminders.
- Automatically collected information. Like most websites, ours records basic technical data such as browser type, device type, general location, and pages visited. This is used in aggregate to understand site usage.
Text Message Program
If you consent to receive appointment reminders by text, we collect your mobile number solely to send those reminders and to process your replies, for example replying C to confirm a visit or STOP to opt out.
Appointment reminder texts contain the minimum information necessary: that an appointment is scheduled, its date and time, and how to confirm or reschedule. They do not contain a diagnosis, treatment details, clinical information, or a child’s name.
You may stop reminder messages at any time by replying STOP. Full program details are in our SMS Terms.
How We Use Information
- To respond to your questions and scheduling requests
- To provide and coordinate physical therapy care
- To send appointment reminders, if you have consented to them
- To meet our legal, professional, and recordkeeping obligations
- To maintain and improve the website
We do not use your information for advertising, and we do not sell it.
How We Share Information
We share personal information only:
- With service providers who help us operate, such as our messaging, scheduling, and records systems, and only to the extent needed to perform that service
- When you direct us to, such as sharing records with another provider, a regional center, or your insurer
- When required by law, or to protect someone’s health or safety
We never share mobile numbers with third parties for marketing purposes.
Children’s Privacy
TTM provides care to infants and toddlers, but our website is directed to parents and caregivers. We do not knowingly collect information online directly from children. Information about a child patient is provided to us by a parent or legal guardian and is handled under our HIPAA Notice of Privacy Practices.
How We Protect Information
We use reasonable administrative, technical, and physical safeguards to protect the information we hold. Clinical records are maintained in access-controlled systems. No system is perfectly secure, and we cannot guarantee absolute security.
Your Choices
- Text messages. Reply STOP to any reminder to opt out, or HELP for help. You may also ask us to remove your number at any time.
- Email. Use the unsubscribe link in any non-clinical email, or contact us.
- Records. To request access to or correction of your health information, contact us using the details below. Rights concerning protected health information are described in our HIPAA Notice of Privacy Practices.
Declining text messages does not affect your child’s care in any way.
Changes to This Policy
We may update this policy from time to time. Material changes will be reflected in the effective date above.
