- Only your first name and last initial are ever published.
- Nothing goes live without the consent checkbox at the bottom of the form.
- You can reply to the confirmation email anytime to edit or pull your review.
- Please don’t include anything you wouldn’t want public — diagnoses, last names, addresses.
Your Review
Your private answers stay private. I use them only to measure outcomes and improve care — I never publish them, never sell data, never share with anyone outside Terran Through Milestones. If you want a copy of what you submitted, reply to the email confirmation and I’ll send it over.
